Rethinking Code Ownership in the Age of AI
The CODEOWNERS file is one of those things in software engineering that everyone uses, nobody loves, and few people question. It’s been the standard mechanism for ensuring the “right people” review code changes for years. And for a while, it was the best tool we had.
I don’t think it is anymore.
Not because code ownership is a bad idea, but because the implementation — static file-based ownership with mandatory human review — is a blunt instrument applied uniformly to problems that vary wildly in shape and risk.
In a world where AI is fundamentally changing the velocity of code production, that blunt instrument is becoming a bottleneck we can’t afford.
The Blunt Instrument
CODEOWNERS has two core flaws that compound each other.
It treats all contributions the same. A one-line string fix and a refactor of your authentication layer both trigger the same review requirement if they touch the same file. There’s no sense of proportion, no concept of risk. The ceremony is identical whether the change is trivial or dangerous.
It treats all contributors the same. A principal engineer with fifteen years on the system and an intern in their first week both need the same code owner approval. The process encodes zero trust in the person making the change. Everyone is equally suspect.
These two flaws create a system that is maximally bureaucratic and minimally intelligent. It optimizes for coverage — making sure every change is seen by someone — without asking whether that coverage is actually producing value.
The Hidden Costs
Beyond the obvious friction, CODEOWNERS creates several second-order problems that quietly erode engineering culture.
People bottlenecks. When a code owner goes on vacation, gets sick, or leaves the company, you have a hard blocker on merging. Some teams mitigate this with group ownership, but that dilutes the very expertise the system is supposed to leverage.
Knowledge hoarding. If only Alice “owns” the payment module, others stop investing in understanding it deeply. Why would they? Alice will always be the reviewer. The ownership model creates learned helplessness — the opposite of shared understanding.
Reviewer fatigue. This is the big one. When you require code owner review on every trivial change, you train reviewers to skim and approve without deep thought. They develop a muscle memory of “looks fine, approve.” So when the actually dangerous change comes through, they’re already in rubber-stamp mode. The signal-to-noise ratio works against you. You’ve conditioned your reviewers to disengage.
The accountability illusion. CODEOWNERS implies a model where the reviewer shares responsibility for the quality of a change. But in practice, when an incident occurs, nobody asks “who approved this?” They ask “who wrote this?” The approver carries zero consequences. If the person with their name on the approval isn’t actually accountable when things break, what is their real incentive to do a thorough review? The answer, for many, is not much. The green checkmark becomes a chore, not a genuine quality gate.
Put it all together and you get a system that feels rigorous — there’s a formal approval, a named reviewer, a green checkmark — but the actual quality assurance happening behind that ceremony can be paper-thin. It’s security theater for code quality.
The AI Layer That Doesn’t Quite Fix It
Many organizations are now bolting AI code review on top of the existing process. An AI reviewer scans the PR, leaves comments about potential issues, and gives its assessment before the human reviewer weighs in.
These tools are genuinely useful for catching mechanical problems — unused variables, potential null references, style violations, obvious bugs. The technical correctness layer is real.
But they have a critical weakness: they lack organizational context. The AI has no idea why this change exists, whether it aligns with the team’s architectural direction, whether it duplicates work happening in another PR, or whether the approach conflicts with a decision made in a Slack thread two weeks ago. It can tell you the code is correct. It can’t tell you the code is right.
And here’s the subtle danger — when the AI review comes back clean, the human reviewer’s guard drops further. Their mental framing shifts from “let me review this code” to “the AI already reviewed it, I just need to sanity check.” That’s reviewer fatigue compounded by automation bias. The green checkmark from the AI becomes a cue to disengage even more.
So you end up with two reviewers — one AI, one human — and neither doing the job particularly well. The AI lacks context. The human lacks attention. The process got heavier, not smarter.
The Velocity Problem
All of this was already straining under human-authored code. But AI coding assistants have changed the equation entirely.
A single engineer with AI tooling can now produce several times the PR volume they could before. The code tends to be verbose, syntactically correct, and “looks right” at a glance — exactly the kind of output that sails through a fatigued reviewer’s skim. The subtle issues — wrong assumptions, edge cases, architectural misfit — are exactly the things you miss when you’re in approval mode rather than review mode.
The production side of the pipeline is accelerating. The review side is not. The human reviewer has become the tightest bottleneck in the entire coding pipeline — the single constraint that most limits how fast teams can ship.
CODEOWNERS was designed for a world where code production was human-limited. That world is changing. The review model needs to adapt accordingly.
From Static Gates to Dynamic Assessment
So what replaces it?
I think the answer is a system that is dynamic rather than static — one that assesses each change on its own merits rather than applying a uniform gate to everything.
Imagine an AI system that isn’t just a technical reviewer, but instead acts as a risk assessor. It analyzes the PR — the size of the diff, the sensitivity of the files touched, the complexity of the logic, the test coverage, the author’s history with this part of the codebase — and categorizes the change by risk level.
Low risk — typo fixes, config changes, well-tested string updates, simple dependency bumps. The AI approves it automatically. The author ships it. No human gate required.
Medium risk — the system flags it, suggests a reviewer (intelligently, based on recent activity and expertise, not a static file), but doesn’t require approval. The reviewer is a resource, not a gate.
High risk — architectural changes, security-sensitive code, core business logic, changes with low test coverage in critical paths. The system requires one or more expert reviewers and suggests specifically who, based on actual recent knowledge of the code rather than a stale ownership declaration.
This is fundamentally different from the current model. It trusts engineers by default and escalates only when the change warrants it. The review requirement becomes proportional to the risk, not uniform across everything.
The Cultural Shift
But tooling alone won’t get us there. This requires a cultural shift in how engineering organizations think about quality and trust.
The current culture around code review is essentially defensive. The unspoken assumption is code is guilty until proven innocent. Every change is suspect until a designated human blesses it. And CODEOWNERS encodes that defensiveness into the workflow.
The shift I’m proposing is moving from review as a gate to review as a resource. Instead of “you cannot merge until someone approves,” it becomes “here’s the level of support this change warrants, and here’s how to access it.”
And here’s the question that makes or breaks this: Can your organization handle the reality that some bugs will ship without a reviewer catching them?
The answer is: they already do. We already ship bugs. Every organization does. The current process lets bugs through — it just lets them through with ceremony. You had a reviewer, you had an approval, the bug shipped anyway. The only difference is everyone feels better about it because the process was followed.
If you look honestly at what catches production bugs today, it’s often not the PR review. It’s the test suite, the CI pipeline, the canary deployment, the monitoring alerts. The engineering investments that actually prevent and mitigate incidents are all upstream of the review step. The infrastructure should be doing the heavy lifting. Organizations should be investing in the systems that actually prevent and mitigate incidents, rather than the review process.
So the honest version of the current system is: we slow down every single change to pass through a human gate that catches a small percentage of issues, while the automated systems catch the majority. That’s a bad trade when velocity matters.
Where This Leads
The PR review should become what it probably should have always been — a knowledge-sharing and mentorship tool. Something you engage when you want a second opinion on an approach, when you’re new to a part of the codebase, when you’re making a genuinely risky change and want expert eyes.
Not a mandatory gate on every commit.
The safety investment should flow toward the systems that actually catch problems — better test coverage, better observability, faster rollbacks, staged deployments, feature flags. These are the real safety nets. The review checkbox was always a comforting illusion layered on top of them.
This is a shift from a prevention culture to a detection and recovery culture. It’s a shift from static, uniform control to dynamic, intelligent assessment. And the AI tooling landscape is finally making it viable.
We’ve accepted CODEOWNERS as the best available solution for a long time. It was. But the world it was designed for is changing fast, and it’s time to build something smarter.
